The Toledo-Lucas County Health Department runs the local vital statistics office at 635 N. Erie St., Toledo, OH 43604. It states that it can provide certified copies of birth certificates for any person born in the State of Ohio after 1908, at $25 per certified copy, so you do not have to go back to your county of birth. The department takes requests in person, by phone and by mail.

Notarizing a translation for Ohio

The Secretary of State states that it does not translate documents and that a translation needing an apostille must be notarized first. Because Form 8003 only accepts notarizations by an Ohio notary public, we hire a notary commissioned in Ohio to notarize the translator’s statement whenever a translation must be notarized for Ohio, such as for an Ohio apostille.

Ohio law also limits what notaries may claim. Under Ohio Revised Code 147.142, a notary who is not a licensed attorney may not advertise as an immigration consultant or use the phrase ‘notario’ or ‘notario publico’ in advertising.
