Why Salt Lake City cannot apostille your FBI background check
Utah has no Secretary of State. Apostilles are issued by the Office of the Utah Lieutenant Governor, whose authentications office is at 350 N State Street, Suite 220, in Salt Lake City. It authenticates original Utah public documents and properly notarized documents, including birth, marriage and death certificates and court judgments.
That office states plainly that FBI background checks and certified documents from federal agencies must go to the U.S. Department of State. It also does not authenticate documents from other states or documents notarized by an out-of-state notary. For an FBI Identity History Summary, the only competent office is the Department of State Office of Authentications in Washington, DC.
Two points that trip up Utah clients:
- A Utah BCI record is not an FBI check. The Bureau of Criminal Identification in Taylorsville, part of the Department of Public Safety, issues a “Right of Access” copy of your Utah criminal record for a $20 fee. BCI’s own site treats the nationwide FBI history as a separate request and points applicants to the FBI for it. If the consulate wrote FBI, federal or national, the BCI record will not meet the requirement.
- Searching for the “Utah Secretary of State” wastes time. People moving here from other states look for an office that does not exist. Even once you find the Lieutenant Governor’s office, it is the wrong place for a federal document.

How it works from Utah
- Get fingerprinted near home. Either have an ink FD-1164 card rolled at a police department, sheriff’s office or fingerprinting vendor, or register on the FBI’s eDO site and give electronic prints at a participating U.S. Post Office. Then request your FBI Identity History Summary. The FBI fee is $18.
- Receive the FBI result. It arrives as a PDF carrying the FBI watermark and the signature of a division official.
- Email us the PDF. We check the name and date of birth against your passport before anything is filed, so include a scan of the passport photo page.
- Hand delivery in Washington. We prepare Form DS-4194 and our Herndon, Virginia office hand-carries the request to the Office of Authentications. There is no DOJ certification step for FBI results. They go directly to the Department of State.
- Return shipping. FedEx to your Utah address, or FedEx or DHL straight to your address overseas.
