Getting a Rhode Island apostille from Warwick
The apostille office is in Providence, not Warwick: RI Department of State, Business Services Division/Apostille, 148 West River Street, Providence, RI 02904. You can mail the request or deliver it yourself, with the order form, the original and $5 per document. Stated processing is 3 to 5 business days from receipt, and documents come back by first-class mail unless you enclose a prepaid courier label.
Two rules catch people out. First, a certificate from Warwick City Hall cannot be apostilled, because the Department of State only authenticates vital records issued by the Department of Health’s Center for Vital Records (6 Harrington Road, Cranston) or the State Archives. Second, a translation qualifies only if a Rhode Island notary other than the translator notarizes the translator’s signed accuracy statement, with the certificate in English and original ink signatures.
For translations that need notarization for Rhode Island, including those headed for a Rhode Island apostille, the translator’s statement is notarized by a Rhode Island-commissioned notary whom we hire for that purpose.

Its FAQ states that transcripts from any institution outside the United States must be assessed by a U.S. evaluation company, such as the Center for Educational Documentation or a current NACES member. The evaluation has to contain an equivalency statement, a course-by-course breakdown with grades and credit hours, and an English translation, and the agency must send it straight to the college.
